Treatment of Recurrent and Habitual Dislocation of the Patella using Madigan¢¥s Procedure in Young Age Group



Abstract
Recurent dislocation of patella is a relatively uncommon disease. The principle of treatment of the recurrent dislocation of the patella is the realignment of the extensor mechanisms. Many surgical procedures of more than one hundred are
introduced
but
it is difficult to select adequate surgical procedure.
We experienced with Madigans procedure and lateral retinacular release as a soft tissue procedure on seven knees in seven patients before epiphyseal closure at Department of Orthopedic Surgery in Kyungpook National University Hospital from
January
1993
to February 1994 and the following results were obtained.
1. The follow-up period was 12 months to 23 months(mean 16 months).
2. The age at operation was ranged from 4 to 15 years, most of them were between 11 and 15 years, and mostly girls(6 knees).
3. The presenting signs were genu valgum in 4 knees and apprehension test positive in 6 knees.
4. The common radiologic findings were lateral patellar tilt in 7 knees, shallow femoral grooge in 5 knees, hypoplasia of lateral condyle of femur in 12 knees and hypoplasia of lateral facet of patella in 2 knees.
5. The mean congruence angle was +1.0 degree (normal side:-9.1 degrees) and mean sulcus angle was 152.9 degrees (normal side: 139.2 degrees).
6. The results according to Madigan¢¥s criteria were excellent in 2 knees, good in 2 knees, poor in 1 knee.
7. The complication was rredislocation in one knee at postoperative 3 months.
8. We thought that Madigan¢¥s procedure as soft tissue procedure of proximal realignment was one of the effective operative method in treatment of the recurrent dislocation of patella in young age group before epiphyseal closure.
